A Medicare-focused regional plan
SCAN's roots are in serving older adults, and its public product materials concentrate on Medicare Advantage. Its 2025 corporate materials described membership in 22 counties across Arizona, California, New Mexico, Nevada and Texas. For 2026, Washington government landscapes independently show SCAN HMOs in Pierce, Spokane and Thurston Counties. Those facts establish a six-state operating footprint, not blanket statewide availability.
The practical question is: “What is offered at my permanent residential address?” Enter the ZIP code, select the county and read the exact plan documents. A neighboring ZIP may have different choices.
SCAN's official document pages provide Evidence of Coverage and Summary of Benefits files. These should be reviewed with the provider directory, pharmacy directory and formulary. A plan name alone cannot answer whether a doctor participates, a prescription is covered or prior authorization applies.
Four clear answers
SCAN is a Medicare Advantage carrier. It is not Original Medicare, and current research did not establish SCAN as a Medicare Supplement or stand-alone Part D issuer.
SCAN's standard plans are generally HMOs. Members usually receive nonemergency care through the plan network and follow the plan's coordination and authorization rules.
SCAN Special Needs Plans have additional eligibility rules. A D-SNP, C-SNP or institutional plan requires more than having Medicare and must be offered at the beneficiary's address.
Benefits differ by plan. Dental, vision, hearing, fitness, transportation, OTC or other benefits cannot be assumed from the SCAN brand.

Understanding SCAN Medicare Advantage
Original Medicare is the federal program. Part A helps cover inpatient hospital care, and Part B helps cover outpatient and medical services. A beneficiary can remain in Original Medicare and consider separate Part D and Medigap coverage, subject to eligibility and underwriting rules.
Medicare Advantage, or Part C, is offered by Medicare-approved private organizations such as SCAN. It administers Part A and Part B benefits and often includes Part D. Plans must cover Medicare-required services, but they can use provider networks, prior authorization and plan-specific cost sharing.
An HMO generally uses a defined network and may require a primary care provider or referrals. Emergency and urgently needed care have special coverage rules. Before enrolling, verify every doctor, hospital, pharmacy and prescription using current plan resources—then call the provider as an added check. Provider participation can change.
Medicare Advantage plans have annual maximum out-of-pocket limits for covered Part A and Part B services. The limit, premium, deductible, copays and coinsurance differ by plan. Prescription coverage follows a formulary with tiers, utilization rules and network pharmacies. Never infer these details from another SCAN plan, even in the same state.

Special Needs Plans and assistance programs
A D-SNP is designed for people with Medicare and qualifying Medicaid. Some SCAN products may coordinate both programs closely, including a Fully Integrated Dual Eligible Special Needs Plan in defined California areas. The state Medicaid agency determines Medicaid eligibility, and the plan determines whether a Medicaid category meets its enrollment rules.
A C-SNP serves people with one or more plan-defined chronic conditions. Diabetes, cardiovascular disorders and chronic heart failure are common Medicare C-SNP categories, but they are examples—not a promise that SCAN has a matching plan in a given county. The plan must confirm the condition after enrollment according to Medicare rules.
An I-SNP or institutional-equivalent plan is for people who meet specific institutional or level-of-care requirements. It is not simply a plan for anyone who receives home care. Confirm the setting, clinical eligibility, participating facility or provider group, county and current contract.
Medicare Savings Programs are administered by states and may help eligible people with Medicare premiums or cost sharing. LIS and Extra Help are the same federal Part D assistance program. Extra Help can reduce eligible beneficiaries' Part D expenses, but it is not a SCAN benefit and an agent cannot approve eligibility.
Part B reductions and supplemental benefits
SCAN plan documents may show dental, vision, hearing, fitness, transportation, over-the-counter or other supplemental benefits in certain service areas. Some plan designs may use a card or allowance mechanism. Only the exact Summary of Benefits and Evidence of Coverage can establish what applies.
Part B premium reductions, flex cards, and other supplemental benefits are not included with every Medicare Advantage plan. Availability, eligibility, benefit amounts, approved expenses, and participating locations vary by plan and service area.
A Part B giveback reduces the amount the member pays toward the Part B premium. It is not cash. A flex card can be used only for approved categories under plan rules. Retailers, providers, eligible expenses, frequency and rollover rules may differ. Benefits connected to chronic-condition or Medicaid status can require additional eligibility.
Veterans, VA care and TRICARE
VA health care, TRICARE and Medicare remain separate. Joining a SCAN plan does not replace VA benefits and does not enroll anyone in VA care. Veterans should examine whether the plan includes Part D, how VA and non-VA prescriptions will be filled, which community providers participate, and how referrals, prior authorization, emergencies and urgent care work.
TRICARE for Life generally coordinates with Medicare in specific ways. A beneficiary should check with TRICARE before changing coverage or enrolling in another drug plan. Military and federal benefit administrators are the authorities on whether existing prescription coverage is creditable and how a change may affect coordination.
What SCAN is not documented to offer
Current research did not establish a SCAN Medicare Supplement policy, stand-alone Part D plan, individual ACA plan, life policy, final expense policy, hospital indemnity plan, critical illness policy, accident plan or stand-alone dental/vision policy. This page should not borrow those products from another carrier or imply they are included in Original Medicare.
