A local comparison starts with your ZIP code and county

Molina's national name does not make every Molina product available nationwide—or even throughout a listed state. Medicare Advantage plans use defined service areas, often at the county level. A D-SNP can also require a particular Medicaid category. Your permanent residential ZIP code, county, Medicare status and Medicaid status should therefore be confirmed before a quote or comparison begins.

Your own personal Medicare agent broker can serve as a consistent contact for these moving parts. Before discussing personal Medicare Advantage or Part D recommendations, the agent should complete a needs analysis and the federally required Scope of Appointment. The needs analysis records practical coverage needs: doctors, hospitals, prescriptions, pharmacies, expected care, financial priorities and other coverage. The Scope of Appointment documents the Medicare product categories the beneficiary has agreed to discuss and helps the agent comply with federal marketing requirements. Signing it does not require enrollment.

Provider access and pharmacy participation can differ within one state. Review costs, networks, formularies, authorizations and benefits every year.

This list does not prove that Molina serves every county or offers the same product in each state. Molina added Iowa and expanded to additional counties in several existing states for 2026, but exact service areas must be verified using current plan documents and CMS landscape data. Hospital indemnity, critical illness, accident, dental, vision, life and final expense policies are also separate from Medicare. They should never be presented as Molina Medicare benefits unless a current official plan document specifically includes a related supplemental benefit.

Molina Medicare Advantage HMO guidance

An HMO generally expects members to use contracted providers except for emergencies, urgently needed care and other situations stated in the Evidence of Coverage. Members may select or be assigned a primary care provider, and some specialist services may require coordination or referral. Exact rules vary by plan.

Before enrolling, verify:

  • Each primary care doctor, specialist, hospital, clinic and medical supplier.
  • The pharmacy network and whether a preferred pharmacy may reduce costs.
  • Every prescription's formulary tier, prior authorization, step therapy and quantity limit.
  • Premiums, deductibles, copayments, coinsurance and the plan's annual maximum out-of-pocket limit for covered Part A and B services.
  • Prior authorization for hospital care, procedures, imaging, rehabilitation, medical equipment and other expected services.
  • Rules for travel, emergency care, urgent care and care outside the service area.
  • The exact eligibility and usage rules for any supplemental benefit.

Molina D-SNP guidance for dual-eligible beneficiaries

Dual eligibility means a person has Medicare and also qualifies for Medicaid. A Molina Dual-Eligible Special Needs Plan may help coordinate the two programs where Molina has an approved D-SNP contract, but enrollment is never automatic.

Eligibility depends on:

  • Enrollment in Medicare Parts A and B.
  • Residence within the plan's approved county and service area.
  • The Medicaid eligibility category required by the particular plan.
  • Continued satisfaction of plan and program requirements.

Medicaid levels matter. One member may receive different assistance with premiums and cost sharing than another. Some supplemental benefits may also depend on eligibility or a health-related condition. Verify Medicaid through the applicable state Medicaid agency. An agent may help explain plan documents and the enrollment process but does not decide government eligibility.

C-SNP and I-SNP availability

A C-SNP requires an eligible chronic condition, clinical confirmation and a plan offered in the county. Examples can include diabetes, certain cardiovascular conditions or chronic heart failure. An I-SNP requires institutional or equivalent level-of-care status. Sources reviewed do not support broad 2026 Molina C-SNP or I-SNP claims; verify the current county.

Help with Medicaid, Medicare Savings Programs and D-SNP coordination

Medicare Savings Programs may help eligible people with the Part B premium and, depending on the category, certain Medicare cost sharing. QMB, SLMB, QI and QDWI are federal categories, but state rules vary. Verify Medicaid and MSP status through the state agency; Social Security handles federal Extra Help applications. Neither Molina nor an agent decides eligibility. Bring current Medicaid information when comparing a D-SNP.

LIS and Extra Help for prescriptions

LIS and Extra Help are the same federal Low-Income Subsidy program. Eligible people may receive help with Part D premiums, deductibles and prescription cost sharing; qualification is not automatic. Part B giveback and flex card benefits

A Part B premium-reduction benefit, sometimes called a Part B giveback, is a plan-specific reduction in the amount a beneficiary pays toward the Medicare Part B premium. It is not “free cash,” does not erase the requirement to maintain Part B and is not available with every plan.

A flex benefit card is plan-specific as well. The plan defines the allowance, eligible categories, participating locations, frequency and usage rules. A card may be restricted to certain dental, vision, hearing, over-the-counter or other approved expenses. Food, utility or broader benefits may require additional eligibility conditions.

Part B premium reductions, flex cards, and other supplemental benefits are not included with every Medicare Advantage plan. Availability, eligibility, benefit amounts, approved expenses, and participating locations vary by plan and service area.

Dental, vision, hearing, transportation, fitness, OTC and other supplemental benefits may also carry network, frequency, authorization or usage restrictions. Read the plan's current official documents.

Medicare guidance for veterans and military families

VA health care, TRICARE For Life and Medicare are separate. Medicare Advantage does not replace VA benefits. Confirm non-VA providers, prescriptions, referrals, emergencies and military-benefit effects with the VA or TRICARE. TRICARE For Life generally requires Medicare Parts A and B, subject to TRICARE rules. An agent can explain Medicare plan terms but cannot change or guarantee military benefits.

Other health and supplemental insurance

Molina offers Medicaid managed care and Marketplace coverage under selected state contracts; neither uses the Medicare footprint above. Licensed agents may also compare individual/family health, life, final expense, dental, vision, critical illness, cancer/heart attack/stroke, hospital indemnity and accident insurance when available. Each has a separate contract and purpose, and none is part of Original Medicare.

Does Molina offer Medicare in Virginia in 2026?

Who may qualify for a Molina D-SNP?

A person must have Medicare, the required Medicaid category and residence in the service area. Qualification is not automatic.

What conditions may qualify someone for a C-SNP?

Qualifying categories can include diabetes, certain cardiovascular conditions, chronic heart failure and other CMS-approved conditions. A current Molina C-SNP must be available in the county, and the condition must be confirmed.

Is Extra Help the same as LIS?

Yes. Both names refer to the federal Low-Income Subsidy for eligible people with Medicare Part D.

How does a Part B giveback work?

If included in a particular plan, it reduces the amount the member pays toward the Part B premium. It is not cash and is not offered with every Molina plan.

Does every Molina Medicare Advantage plan include a flex card?

No. Availability, eligible expenses, allowance, participating locations and usage rules depend on the exact plan.

What is the difference between an HMO and a PPO?

An HMO generally relies more heavily on a contracted local network and care coordination. A PPO may permit more out-of-network care at different costs. Confirm which designs Molina actually offers in your county.

Can I keep VA health care and enroll in a Molina Medicare plan?

Some veterans maintain both, but the systems are separate. Confirm prescriptions, non-VA services, emergency care and any military-benefit impact with the VA or TRICARE.

Is there a cost to speak with an O'Neal Insurance Group agent?
