Medicare Chronic Condition & Special Needs Plans.

Are you one of the millions of Americans who has a chronic health condition, such as diabetes or heart disease? If so, consider the importance of a Chronic Condition/Special Needs policy.  The main advantage of these plans is that they often include access to providers who specialize in treating chronic diseases – coverage is tailored to treat your specific needs.

These plans are only available to those with a disabling chronic condition or who need frequent nursing care, whether at home or in an institution, such as a nursing home.

To join, you must be enrolled in Medicare Part A and Medicare Part B.

Diseases that qualify as chronic conditions include, but are not limited to:

  • Dementia
  • Diabetes
  • Chronic heart failure
  • Autoimmune disorders
  • End-stage renal disease (requiring dialysis)
  • HIV/AIDS
  • Stroke
  • Neurologic disorders
  • Chronic lung disorders
  • Chronic and disabling mental health conditions

Do you have one of these conditions and would like to know more? Contact us, and one of our professional agents will provide a confidential assessment and a free quote.
